Requested documents
For scheduled medical care (consultations, tests, treatment and hospitalisation), it is important that you bring the necessary insurance documents. Payment up front is also required.
One of the following documents is essential in order for your medical care to be reimbursed by your insurer.
For European citizens: the S2 form
The S2 form is proof of your right to scheduled health care in another EU country, Iceland, Liechtenstein, Norway or Switzerland. You need to request this document from your health insurer beforehand and submit it to the Services and International Patients desk at UZ Brussel. You can send it in by mail or hand it over to the registration desk when you register.
The following information must be filled in on the S2 form:
- Patient's name and date of birth
- Period of validity
- Type of treatment and medical service
- UZ Brussel as the hospital where the treatment is being provided
With the S2 form, the treatment is provided under the same (financial) conditions as for Belgian insured persons. You pay a percentage of the costs up front.
For non-European citizens: valid cover (payment guarantee) from an insurance company
Are you insured abroad? If you insurance company approves the scheduled medical care at UZ Brussel, then be sure to ask for a document in which they state their consent. You can send it to us by e-mail or hand it over when you register at UZ Brussel.
If you do not have one of the two documents
You will pay the full amount of the estimated treatment costs beforehand as an advance. The amount of the final invoice may differ from the total of the advances paid.
